If you run a clinic, polyclinic, dental practice or diagnostic lab in Dubai, carbonless (NCR) pads are the one print item you cannot improvise. The patient keeps a copy, the file keeps a copy, and in a DHA or DoH inspection the duplicate is the proof that consent was taken. This guide covers the five pads every Dubai clinic orders, the layout rules that matter for medical records, and real 2026 prices — a 5-book run of A5 consent pads starts at AED 265, VAT inclusive.

Quick orientation on the notation you will see when ordering: 1+1 means original plus one copy (a 2-part set — white original, pink or yellow duplicate). 1+2 means original plus two copies (3-part). For most clinical forms 1+1 is correct; 1+2 is for anything where a third party — an insurer, a referring doctor, a lab — also needs a physical copy.

The five pads most Dubai clinics order

1. Consent forms (A4, 1+1)

Procedure consent, anaesthesia consent and photography/media consent. A4 is the right size — the form needs room for a procedure description written in longhand, two signatures, a witness line and the date. Order the A4 NCR book, 1-colour, 1+1 at AED 294 for 5 books. The patient keeps the duplicate; the original is filed.

2. Lab request forms (A5 or A4, 1+2)

These travel. One copy stays with the clinic, one goes with the sample, one goes back with the result. That is a 1+2 set — the A4 full-colour 1+2 NCR book at AED 580 for 10 books. Full colour is worth it here because colour-coded test panels (haematology, biochemistry, serology) cut transcription errors at the bench.

3. Prescription pads (A5)

Most Dubai clinics now issue prescriptions through the practice management system and print on A5 letterhead rather than a carbonless pad — a printed A5 letterhead at AED 150 for 500 sheets is the cheapest and most flexible option. Keep a carbonless A5 pad as the fallback for home visits and system downtime: 5 A5 NCR books, 1+1 at AED 265.

4. Referral and sick-leave forms (A5, 1+1)

A5, 1+1, two-colour if you want the clinic logo to read clearly on both the original and the duplicate. AED 300 for 5 two-colour A5 books. Sequential numbering is essential here — ask for it at order stage.

5. Receipt and payment vouchers

For cash-paying and insurance co-pay patients. Receipt vouchers at AED 145 and payment vouchers at AED 145 are standard-format carbonless books that need only your logo and TRN added. Browse the full range in the NCR books collection.

Layout rules that actually matter for medical forms

Carbonless paper behaves differently from ordinary stock, and medical forms get filled in under time pressure. Five rules save most of the trouble:

  • Keep ink coverage light on the fill-in zone. NCR works by pressure transfer. Heavy solid backgrounds or a dark watermark behind the writing area will transfer unevenly and the duplicate becomes unreadable. Keep tints under 15% where the pen will go.
  • Write-on boxes need 8 mm minimum height. 6 mm looks tidy on screen and is unusable with a ballpoint. For a signature line allow 12 mm.
  • Put the sequential number top-right on every part. It must appear in the same position on original and duplicate so a file audit can match them without unstapling.
  • Print the clinic name, DHA licence number and TRN on every part, not just the original. The duplicate is the one that ends up in an inspection folder.
  • Arabic–English bilingual consent needs mirrored columns, not a translation pasted below the English. Two parallel columns, each readable on its own, is what reviewers expect — and the patient signature line should sit below both.

One more practical point: specify the duplicate colour. White original with a pink duplicate for consent, yellow for lab requests, blue for financial vouchers is a common clinic convention and it lets reception sort loose copies at a glance.

Labels: sample, file and appointment

Clinics buy more labels than they expect. Three types cover most of it:

Sample and vial labels. Small round paper stickers at 5–6 cm diameter take a patient ID and date written by hand. 200 round 5 cm paper stickers at AED 375, or 1,000 at 6 cm for AED 625. For a lab running real volume, 10,000 round 5.5 cm stickers at AED 950 brings the unit cost down to under 10 fils each.

Tamper-evident and equipment labels. Transparent PVC stickers survive wiping with alcohol, which paper does not. 1,000 transparent 5 x 5 cm PVC stickers with glossy lamination are AED 410.

File and shelf labels. Plain 8 x 12 cm unlaminated stickers at AED 410 per 1,000 are the cheapest way to label record boxes and store-room shelving. The full catalogue is in the stickers and labels collection, and material choice is explained in our guide to paper, PVC and vinyl sticker materials.

How to order

  1. Send the form as a PDF or Word file. Most clinics already have a form they are using — send that, say what is wrong with it, and we will rebuild it to the NCR rules above. If you are starting from nothing, send the field list and we will lay it out.
  2. Specify parts and copy colours. 1+1 or 1+2, and which colour the duplicate should be. If you want different colours for different form types, say so in one message so the whole order is consistent.
  3. Specify the numbering range. For example 00001–00500 for a 10-book run of 50 sets. If you are continuing from an existing pad, give us the last number used so the sequence does not restart.
  4. Approve the proof. You will get a PDF showing each part separately so you can confirm that the licence number, TRN and numbering appear on the duplicate as well as the original.
  5. Production: 3–4 working days from proof approval. Delivery across Dubai and Sharjah included; Abu Dhabi and the northern emirates next-day after dispatch.

FAQ

How many sets are in one NCR book?

50 sets per book as standard. A 5-book A4 order at AED 294 therefore gives you 250 consent forms with duplicates — around three to six months of use for a single-doctor clinic, less for a busy polyclinic.

Should consent forms be 2-part or 3-part?

Two-part (1+1) for standard procedure and anaesthesia consent: patient keeps one, the file keeps one. Go to three-part (1+2) only when an insurer or a referring clinic also needs a physical copy — otherwise you are paying for a sheet that gets thrown away.

Can you print bilingual Arabic and English consent forms?

Yes. Set them as two parallel columns so each language reads independently, with the signature and date lines spanning both. Send the Arabic text as a Word file or outlined vector so the ligatures do not break at output — screenshots of Arabic text cannot be used for print.

Do NCR pads come with sequential numbering?

Yes, numbering is included. Tell us the starting number and the range, and keep a record of where each batch ends — the most common reorder problem is a clinic receiving a new batch that duplicates numbers already in the patient files.

What is the minimum order for medical NCR pads?

Five books, which is 250 sets. Below that the setup cost dominates and the price per set stops making sense. If you need several different forms, order five books of each rather than fifty of one — most clinics underestimate how many separate form types they are actually using.

Will the duplicate copy be readable?

Yes, provided the artwork keeps heavy ink coverage out of the fill-in area and the writer uses a ballpoint rather than a gel or rollerball pen. Gel ink glides and transfers poorly. This is worth telling reception once — it is the single most common cause of an illegible patient copy.
